Primary Responsibilities:
- Manage defined caseload utilising specialist knowledge and advanced clinical reasoning.
- Undertake assessment, planning, implementation, and evaluation of care programs for children and families.
- Liaise with and refer to relevant agencies as needed.
- Take an active role in holistic assessment and treatment of young people with mental health problems, particularly those at risk of or who have attempted deliberate self-harm.
- Provide liaison with other professionals and agencies to ensure comprehensive and integrated care.
- Work collaboratively as part of a multi-disciplinary team.
- Offer evidence-based therapeutic interventions in accordance with NICE guidelines.
- Maintain clients' confidentiality rights at all times.
- Uphold highest standards of clinical record-keeping, including electronic data entry, recording, and report writing.
- Exercise autonomous professional responsibility for assessment, treatment, and discharge of children, young people, and their families/carers.
- Provide assessment and therapy services based on integration of complex data from various sources.
- Undertake risk assessment and risk management for individual clients and their families/carers.
- Act as an advocate to facilitate the views and choices of children, young people, families, groups, and communities, as appropriate.
Minimum Qualifications:
Possess a minimum of 6 months' experience as a Community Psychiatric Nurse.
Registered with the Nursing and Midwifery Council (NMC).
